Juror Qualification Questionnaire If you received a Juror Qualification Questionnaire (see form below) from the U.S. District Court, you are being considered for jury service. However, this is not a summons for jury service and you are not being called to report at this time.
The juror questionnaire contains several questions that would normally be asked in person by the trial judge during the jury selection process. By asking these questions in advance, we can reduce the number of potential jurors that we ask to report to the courthouse.

The questionnaire is designed for the Court to determine if someone is qualified to serve as a juror, and we ask that you fill out the questionnaire completely.
