Vanderbilt Medical Record Forms (VMR Forms) is the information hub for all documents that will be entered into the electronic medical record. This encompasses both internal VUMC forms, as well as, non-VUMC external forms used by our staff and clinics.

Vanderbilt University Hospital Medical Records can be requested by mail or fax through downloading the authorization form from the hospital's website, complete it and send back to Center for Health Information Management.
